Ten manufacturers hold 42.7% of Open Payments dollars

42.7%
of 2025 Open Payments dollars reported by the ten largest manufacturers

Ten companies account for 42.7% of every dollar reported to Open Payments by 1,833 manufacturers for program year 2025. The top 50 hold 75.7%. The top 100 hold 85.5%. The largest single reporter is 6.2% on its own. All ten of the largest are drug and biologic makers.

The total is $13.42 billion across 17.06 million transactions. Research payments are $9.50 billion of it. General payments, the meals, consulting, travel and speaking fees a field rep would recognize, are $3.92 billion.

The tail is long and thin. The median manufacturer reported $178,816 for the year. 774 of the 1,833, more than four in ten, reported under $100,000. Everything below rank 1,000 adds up to 0.21% of the total.

For a rep the read is simple. Sell against a top 50 name and your competitor's relationship spend at the account is public, itemized, and probably larger than yours. Sell from the tail and the record shows almost nothing, which cuts both ways: nobody can see you, and the physicians in your territory have already been called on by someone whose budget is a matter of public record.
