High nurse turnover more than doubles the rate of low star ratings

2.3x
rate of one and two star ratings at nursing homes with 60% or higher nurse turnover, against homes under 40%

Among 9,580 nursing homes reporting both a nurse turnover rate and an overall star rating, 20.0% of those with turnover at 60% or higher sit at one or two stars. Under 40% turnover, 8.6% do. That is 2.3 times the rate.

It runs the other way too. 17.1% of low turnover homes hold four or five stars. 3.1% of high turnover homes do. Average rating falls in a straight line across the four buckets: 3.10, 2.95, 2.81, 2.55.

Staffing hours move with it. Low turnover homes average 3.82 total nurse hours per resident day. High turnover homes average 3.56. RN turnover rises in step with total turnover: 30.7% in the low bucket, 67.1% in the high one.

1,664 homes sit in the 60% and up bucket. That is the list. A building replacing two thirds of its nursing staff every year is paying for orientation, agency hours and survey risk, and it is the building most likely to take a meeting about anything that cuts the work per shift.
