Public CMS files cover traditional fee-for-service Medicare. In 2026, 55 percent of Medicare beneficiaries sit in Medicare Advantage and are largely absent from those files, and everybody on commercial insurance or Medicaid is absent entirely. So this tells you who to see and why, from structural signals. It cannot tell you who refers to whom. If that is the question you need answered, you want a claims vendor, and they will price like one.
It does not score zero, it does not guess, and it does not quietly average something in to keep the number tidy. A hospital with no reported measures shows fewer lenses and says so. Modeled figures are badged as estimates every time.
Not by you, not by your manager, not by me. The database refuses UPDATE and DELETE on the event table outright. Tap the wrong account and you fix it the way an accountant does, by posting a visible reversal that sits in the log next to the original, not by quietly deleting the mistake. Every event also freezes the score it was written against, so a later rescore cannot change what your Tuesday looked like.
The data sources page reads publication dates out of the running product, and it separates when the government published a file from when we last pulled it. Pulling a 2023 file this morning does not make it a 2026 file. If something is stale, the page says so instead of saying "always up to date".
